You should consider adding a notice to participants at the start of your screeners if you are going to ask sensitive questions. This is important to ensure your compliance with privacy legislation.
Add it as a description on your first screener question, or as the opening line of the question itself. Here are three patterns to adapt:
Example 1: If at any point you don't feel comfortable answering these questions, you can opt out at any time by deciding not to submit in accordance to our privacy policy.
Example 2: The following questions may be sensitive for some people. If you're in crisis, in danger or having suicidal thoughts, please call Lifeline Australia (13 11 14).
Example 3: Please note the following questions relate to the topic of [replace with your topic] and may be confronting for some. If you don't wish to continue, feel free to close this application.
If your study recruits outside Australia, swap the Lifeline number for a crisis line local to your participants' country. For reference these are the lifeline contacts for the UK and US:
